ABSTRAK Stoma ileostomi mengeluarkan ekskreta cair, bersifat alkali dan mengandung enzim pencernaan yang masih aktif. Sedang kantong stoma plastik yang secara umum telah dipakai untuk menampung gkskreta menimbulkan trauma eksfoliasi, bersifat imunogenik dan kebocoran ekskreta. Hal tersebut secara bersama-sama dapat menimbulkan iritasi pada kulit peristoma. Penelitian ini bertujuan untuk menilai efek proteksi skin barrier batok kelapa terhadap kebocoran ekskreta pada kulit periileostoma. Studi ini membandingkan kadar elastase pankreas-1 pada kulit periileostoma, antara pemakai kantong stoma plastik dan pemakai kantong stoma dengan skin barrier batok kelapa. Jumlah sampel pemakai kantong stoma plastik 8; batok kelapa 9, diambil dengan cara swab pada kulit peristoma, selanjutnya diperiksa dengan metode Elisa. Sampel diambil pada hart ke-15 setelah pembuatan stoma ileostomi. Dengan uji student-t test didapatkan perbedaan bermakna (P = 0,00, CI 95%) antara kadar elastase pankreas -1 pada kelompok pemakai kantong stoma dengan skin barrier batok kelapa (26.38 ± 37.52 ug/dl) dan kelompok pemakai kantong stoma plastik (323.44 ± 135.91 µg/dl). Pemakaian skin barrier batok kelapa dapat mengurangi tingkat kebocoran ekskreta dan iritasi pada kulit stoma ileostomi Kata kunci: Batok kelapa, kantong ileostoma plastik, elastase pankreas-1, kulit periileostoma ABSTRACT Ileostoma produces watery and alkalin excreta, and contains also active digestive enzymes. Ileostomal plastic bags have been commonly used by the ileostomates to collect any excreta produced. However, ileostomal plastic bags often provoke exfoliative trauma, immunogenic reaction and tendency of leakage to the periileostomal skin. Those factors altogether can generate peristomal skin destruction. This research is intended to evaluate the protective effect of coconut shell skin barrier against the excretal leakage on the periileosomal skin. This study compares the concentration of pancreatic elastase-1 enzyme (ELA-1) on the periileostomal skin, between plastic stoma bag users and plastic stoma bag with skin barrier coconut shell users. The number of samples among the plastic stoma bags is 8, and coconut shell skin barriers is 9. Samples are taken by swab, using a spesial spoon from periileostoma skin surface, and proceeded with Elisa method. Samples are obtained on the 15` day after ileostoma procedure. In analysis with student-t test, the concentration of ELA-1 on the periileostomal skin is significant difference (P = 0.00, CI 95%), between the plastic bag with coconut shell skin barrier users (26.38 ± 37.52 µg/g), and plastic bag users (323.44 ± 135.91 µg/g). The use of plastic stoma bag with skin barrier coconut shells is able to lessen the degree of excretal leakage and periileostomal skin irritation respectively.
